Rotator Cuff Pain Symptoms and Management

Rotator cuff pain is among the more debilitating ailments we can suffer because we use our shoulders for almost everything we do. Together with weakness and inflammation, shoulder pain is the most commonly reported symptom of a rotator cuff injury.

The great news is that in all but the most severe cases, pain can be reduced or eliminated by regular adherence to a rotator cuff exercise program.

How do I know if my pain is associated with rotator cuff injury?

It can be very hard to describe pain in the cuff. It may exist merely as a dull ache. Yet even such mild pain may be symptomatic of the onset of impingement. Impingement, over time, can develop into a full-blown tear.

Pain from early onset impingement usually only occurs during exertion of the arm, particularly if this has involved raising the arm overhead. If left untreated, the impingement is likely to worsen, as will the pain.

As the degree of pain increases, so will the duration of episodes, and the anatomical distribution of the pain, radiating from the front of the shoulder down the affected arm. If inflammation is also present in the tendons, this may be felt as a burning sensation within the shoulder.

The most severe pain arises from a full-thickness cuff tear, entailing detachment of the tendon from the humerus. The pain is severe and sudden in onset. It may diminish over time, but will not disappear. This contrasts with chronic injuries, where the pain is the result of an ongoing, persistent insult, and is likely steadily to increase.

What if the cuff actually tears?

If the injury to your shoulder should develop into a full-blown tear, the experience of pain is significantly different from lower levels of injury. When a tear occurs, the sensation of tearing is often palpable. You will feel - again across the front of the shoulder, and usually down the arm as well - an intense pain.

This pain will increase with continued use of the limb. In particular, lifting the arm away from the body or above the head will greatly exacerbate the pain. While the pain from a tear can diminish over some months, the underlying weakness, unless treated, will persist.


Recovery from a rotator cuff injury and permanent freedom from rotator cuff pain; will be greatly aided by the correct use of exercise. The significant advantage of exercise is that it is entirely natural. Exercise builds on the shoulders natural range of motion whilst at the same time developing strength.
